- An up-to-date personal contact information, which (at least) includes your contact number, email, and mailing address.
- A personal statement declaring your firm belief that the copyright-protected content found on our application or other services does not have the consent of the owner, its copyright agent, or the law.
- A statement declaring all information shared on your document is accurate and that (under the potential penalty of perjury) you are the owner of the copyright-protected intellectual property or the person legally authorized to act on behalf of the owner.

All Copyright counter-notice of Alleged Infringement must contain the following information:
- The physical or electronic signature of the person filing the counter-notices.

- A personal statement expressing your consent for the Local Federal Court to provide jurisdiction and your willingness to accept service of process from the author of the Copyright Notice you’re countering.
